If you are a member of a Medicare based HMO, please contact your Health Plan
at the number on your card for assistance.
If you are covered through your employer, please contact your employer as
changes may have to be requested through your employer.

If your Primary Care Physician has submitted an authorization and you have
not received a response, please contact Customer service at 310-417-5900.

has been denied?
